Evaluation of a Pilot Primary Care Network on the Sunshine Coast
The need:
The Ministry of Health, Sunshine Coast Division of Family Practice, Vancouver Coastal Health and shíshálh Nation are working in partnership towards an integrated system of primary and community care through the establishment of a Primary Care Network (PCN) on the Sunshine Coast. In early 2021, the MoH approved funding for a social worker be integrated into family physician clinics as an ‘early draw’ on future PCN funding. The collaborative required an evaluation of this pilot project to identify what worked well, areas for improvement, and lessons learned to inform the expansion of the Coast’s PCN.
What we did:
Catalyst completed a process and outcome evaluation of the PCN pilot, which included assessment of provider experience, progress towards goals, project successes, challenges, and suggestions for next steps. A mixed methods approach was employed, including interviews with diverse project partners and analysis of electronic medical records to assess supply and demand for social work services. Our report provided the PCN Steering Committee with actionable recommendations that supported them to build upon lessons learned from the pilot when introducing future allied health professionals and other primary care resources.
