Outcomes of a Pilot to Support Specialist to Transition to Team Care Models
The need:
In January 2023, the Specialist Services Committee (SSC) implemented a pilot program to support 11 specialist-led teams to implement team care models in their outpatient community practices called the Specialist Team Care Collaborative (STCC). Generally primary goals of this project were to increase clinical capacity, improve the patient and provider experience, and reduce acute care resource utilization. The SSC required a robust evaluation to assess progress towards these intended goals.
What we did:
Working with the STCC Working Group, Catalyst Consulting designed a comprehensive evaluation strategy to measure progress towards the Collaborative’s goals, as well as identify strengths and weaknesses of the pilot’s design. Quantitative data collection methods were used to assess key outcomes before, during, and after the transition to team care models, which incorporated the use of validated tools where possible. A patient survey was employed to assess their experience with the team care models, as well as their use of acute care resources over time. In-depth interviews and focus groups with key groups offered narrative descriptions of progress towards goals. The final evaluation report summarized key findings and provided the SSC with actionable recommendations regarding the future of sustaining and expanding the Collaborative model for specialist team care.
